The Wellness Coordinators are still providing Health Coaching and exercise programs to do at home so that you can stay active while still staying safe.  Call our clinic to book in with Patrick or check out our Health Education tab for current programs provided by Erica Laycock, Chronic Condition Client Health Educator.

LOW RISK OBSTETRICAL PROGRAM

(Deliveries not currently available in Three Hills, but we provide PN care until the 32 week mark)

Our clinic is part of the https://bigcountrypcn.com/.  They provide professional staff as a part of our multidisciplinary team approach to enhance and deliver health care services. Presently we have a Nurse Practitioner, an RN, LPN, Footcare Nurse, a Wellness Coordinator, Referral Navigators and a Mental Health Therapist working with us.

Nurse Practitioner: Kendra Taks has been with our KMC team since September 2022! With years of nursing experience first, then going for her NP degree, she can do almost everything a physician does!  Kendra is no longer accepting new patients, but will see patients who don't have a family doctor if they need to be seen for a particular medical concern.

Nurses: They are responsible for patient education and follow-ups for chronic disease management, health promotion and prevention for all ages. They also assist the physicians with medical procedures and tasks.  We have a nurse doing footcare every Wednesday and on Thursday afternoons. Patients can self-refer to see any of the nurses.

Referral Navigator: The Navigator's role is to facilitate patient referrals to specialists, specialty clinics and other medical investigations.

Mental Health Therapist:  Tim is a very welcome addition to our team!  He provides counselling here at KMC two days a week for a variety of mental health needs.  This is by referral only, so ask your doctor if you need that extra support.

*NEW*Community Liaison Coordinator: At KMC every Thursday!

This position is focused on Seniors and can help with things such as stretching and finding balance exercises for falls prevention or to maintain mobility; memory loss or dementia support; caregiver support and education; decreasing risk for social isolation by connecting people to community services and groups.  Appointments can be made with Kim by calling the clinic at 403-443-5522 and speaking to the patient navigators.
